PO-04-064 QUANTIFICATION OF AUTONOMIC MODULATION DURING SINGLE-SHOT PULMONARY VEIN ISOLATION WITH PULSED FIELD ABLATION
نویسندگان
چکیده
Additional ganglionated plexi (GPs) ablation beyond pulmonary vein isolation (PVI) has been associated with better outcomes after radiofrequency ablation. Because of the anatomical relationship between major atrial GPs and veins, single-shot PVI cryoballoon catheter (CBA) demonstrated to concomitantly affect GPs, as documented by intraprocedural vagal reactions increased heart rates (HR) The novel multispline pulsed field (PFA) is emerging a safe effective non-thermal alternative for PVI, selectivity cardiac tissue. To quantify degree parasympathetic denervation during PFA compared CBA. We prospectively included consecutive patients undergoing autonomic modulation assessment extracardiac stimulation (ECVS) paroxysmal AF Patients previous ablations were excluded. response ECVS was determined sinus pause following stimulation, measuring maximum PP interval (sinus node response). Baseline post-ablation responses collected analysed. in two groups assessed evaluating difference pre- ECVS. A total 30 (PFA group, n=15 vs CBA n=15) analysis. patients’ characteristics similar (Table 1). Pre-ablation (10757.3 ± 3155.2 ms vs. 11155.9 2472.3 ms, p=0.70). Vagal occurred more frequently than (86.7% 20%, p=0.001), commonly left superior PVI. After significantly longer (9897.9 2702.0 874.6 213.6 ms; p<0.001, Figure Despite baseline values, 24h HR (14.0 6.8 bpm 1.8 8.4 bpm, respectively; p<0.001). Single-shot less at testing. This likely result tissue selectivity, minimal effect on nervous system. clinical impact reduced outcome yet be elucidated.

Functional Pulmonary Vein Isolation During Atrial Fibrillation Ablation.
We report a case of apparent isolation of the right inferior pulmonary vein during atrial fibrillation ablation coincident with the onset of an atrial tachycardia. This report highlights the importance of assessing pulmonary vein conduction during sinus or paced rhythm at relatively long cycle length, rather than in atrial fibrillation or atrial tachycardia.

This study was designed to determine whether endocardial high-frequency stimulation at the pulmonary vein (PV) antrums can localize cardiac autonomic ganglionated plexi (GP) and whether ablation at these sites can evoke a vagal response and provide a long-term benefit after PV isolation (PVI) for atrial fibrillation (AF). Radiofrequency ablation of each PV antrum was performed in 21 patients wi...
متن کاملThe Autonomic Nervous System and Atrial Fibrillation:The Roles of Pulmonary Vein Isolation and Ganglionated Plexi Ablation.
After the sequential successes of catheter ablation for the treatment of pre-excitation syndromes (WPW), junctional reentry (AVNRT) atrial flutter (AFL) and ventricular arrhythmias, clinical electrophysiologists have focused on the myocardial basis of atrial fibrillation (AF). Thus, the strategy for ablation of drug and cardioversion refractory AF was to isolate the myocardial connections from ...
متن کاملDissociated pulmonary vein potentials: Expression of the cardiac autonomic nervous system following pulmonary vein isolation?
Introduction In the electrically nonisolated pulmonary veins, the cardiac autonomic system has been shown to play an important role in initiating pulmonary vein (PV) ectopy and triggering atrial fibrillation (AF). However, the effects of the cardiac autonomic system on the isolated PV are not currently known. We present the observations from a case where opportunistic stimulation of the autonom...
متن کاملPrevalence, characteristics, and predictors of pulmonary vein narrowing after isolation using the pulmonary vein ablation catheter.
BACKGROUND The risk of pulmonary vein narrowing (PVN) after pulmonary vein isolation, using a novel multi-electrode ablation catheter, is unknown. METHODS AND RESULTS Left atrial volume and PV diameters were compared by computed tomography (CT) before and 3 months after pulmonary vein isolation using duty-cycled phased radio frequency energy (2:1 or 4:1 bipolar/unipolar ratio) in 50 patients....
